Well, I had this issue on my dedicated server, seemingly out of nowhere, which means cPanel did something... Anyway, I tried many thing but what worked finally was tricking Outlook into thinkin it had brand new messages to download, not just a server full of messages it had already downloaded. I did this because when I created a new account it worked, which meant our server sent out something to the Outlook user telling it that the messages on the server had all been downloaded, even though new ones were coming in every minute. So this is what I did that worked:
using Horde, I copied all the messages out of the inbox, changed the outlook settings to download the messages and not leave a copy on the server (which just deleted the messages, didn't download them), then I changed the Outlook settings back to leaving them on the server for 7 days, then I copied the messages that hadn't been received yet back to the Horde inbox. The next Send & Receive saw all the messages copied back there as never having been downloaded (even the 10 extra that had been downloaded before the problem arose). So it worked! I think this is related to the maildir conversion, but I don't really know since it's more of a trick than a fix... I hope this helps someone else!
